I think as far as redpills go, once out of the system, people would lose a considerable amount of weight(assuming they were overweight in the first place) from both the trauma and shock of extraction, as well as the lack of readily-available food in the known real world. The reconditioning of the muscles after extraction would play a part in a person's physique as well. Also, what Nighttrace said about the Residual Self-Image.
As far as bluepills, I would think there would be people that are overweight or obese. I think that the machines would have programs doing what we see in our society today; using subliminal imagery in entertainment and advertising to encourage them to be more "efficient" and exert more energy.

I imagine that, as a plugged in Bluepill, you get fat as a normal process of food intake. But in the real world, it makes more sense that the Machines would euthanise any humans who aren't producing their optimum amount of energy--so people have heart failure, clogged arteries, heart attacks, etc (Yay for conspiracy theories)
And then, what Pyraci said is also true, once a human is freed from the Matrix, they find themselves having never really eaten anything or exercised their muscles at all, so they're bound to be pretty weak and atrophied. And finally, NT is right, we don't want to imagine ourselves as less than perfect if we had the chance, so we create a Residual Self Image of ourselves at the peak of ourselves.

In this reality Colonel Sanders was promoted to lead the Machines before he invented his finger licking recipies, hence the whole fast food industry never existed.

Genetic engineering. Grown people are grooooooown. Why would the Machines create a crop of people that introduced risk factors (obesity causes so many potentially fatal health problems) into the equation? It would be their most sincere desire to delete such variables.

Because the Matrix has to seem real to bluepills in order for them accept the matrix. If everyone is in tip-top shape and perfect health, wouldn't you start to question things?
And as far as RSI goes, it's supposed to be "the mental projection of your digital self", not the projection of what you want yourself to look like. Whether or not that can be manipulated in that way has never really been talked about as far as I know (please correct me if I'm wrong).
Now really, I know the simple answer is that overweight people in movies don't do kung fu. But I thought it would be interesting to talk as if that weren't a factor, and I enjoy some of the theories people have come up with.

An in-shape, athletic human generates more energy than a sedentary, out of shape, obese one. It would be inefficient to have overweight people plugged in.

Correct but how many of those in-shape athletic humans are really in-shape or athletic? Obesity just like everything else inside the Matrix is just apart of the illusion. When it comes down to it Fat Albert is really a skinny bald shriveled up body laying inside a pod.
As to why all redpills seem to be buffed and not overweight well that may just come from the idea that all redpills are soldiers and are required to be in-shape. Speed and strength in the matrix may have nothing to do with the mussels but in the real world its all you have.

Redpills, as protagonists in a computer game, are supposed to be young, fit, intelligent people. Not to mention well-shaped RSIs are easier to animate and more likely to be used as avatars.
