This problem still pops up fairly frequently.
The problem is caused by your video card not being able to run the game properly.

To fix the issue:

Ensure your PC meets the minimum specifications for the game
Update your video card drivers to the latest version

If the problem persists
Run the game in windowed mode by adding the following line to the end of your "options.cfg" file in your Matrix Online install folder:

windowed = 1
