It wouldn't theoretically have to be done server side, The models could be saved on your computer and then loaded to the Servers each time you activated the model therefore not requiring any sort of excessive modifications to the database. Like how /savelo is used whereby it saves what abilities you have loaded as each apparel model will have a unique Hexidecimal value which could be saved to a file on the players computer to be loaded with a command into the server to be displayed to all the other players.
The only thing I would want to see is probably an indicator when someone is using that because in the recent Vector Cup tourney it would be easy to make a clothing setup which contained normal Ballistic accuracy clothing but then under that wear Sleepwalker pants ect.
